If you’ve called for emergency medical services from the City and were assisted or transported by Berkeley paramedics, you can access available documents and resources.
Get patient care reports and AmBULance billing records, INCLUDING DEPOSITIONS
A patient care report (PCR) is generated whenever the Berkeley Fire Department assists a sick or injured person.
Copies of PCRs are available to our patients and/or their authorized representatives. These reports contain confidential medical information and are not part of the public record. These records are kept in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
Call Wittman Enterprises at (800) 906-6552 for:
- A copy of a PCR for yourself, a dependent or a client (attorney or subpoena)
- A copy of ambulance billing records for yourself, a dependent or a client (attorney or subpoena)

The Fire Department contracts with Wittman Enterprises to provide billing services for all of our ambulance transports.

The existing EMS and Fire Recovery Billing Rates:
- Bundled Rate: $4,529
- Mileage: $102.21
- Oxygen: $338.42
- Treatment / Non-Transport: $908.49

Keep your personal medical information easy to find in a common area in your home, wallet, and car.
Include the following information:
- Name, date of birth, and address
- Emergency contacts and phone numbers
- Medical information: primary doctor, medical conditions, medications, allergies, preferred hospital, insurance information, and any special needs or disabilities
- Communication needs, language
- Other important medical notes
